Minute Hand Drops

Can anyone offer help? Lately when I assemble my clocks for no aparent reason the minute hand drops and hangs down. This is without battery.
Just sitting. I am quite sure I am tightening nut enough. Any help is appreciated. Any tips for installing hands is also appreciated
